Information Required:                  

  • Personal Identification for each applicant (1 Photo-ID or 2 other ID’s)

  • Identification for all children claimed (must be original ID, no photocopies)

  • Proof of current address (You must live in Oshawa or Port Perry to apply)

  • rent receipt or lease

  • Government Assistance Stub or Pay Stub for everyone in the household

Hamper Pick up:
